Bradley Creek Health Center, located on the 24-acre campus of Carolina Bay within Wilmington’s Autumn Hall mixed-use community, offers a full range of health care services to meet the needs of the individual. From Inpatient and Outpatient Rehabilitation to Assisted Living, Memory Support*, and Skilled Nursing Care, the staff of Bradley Creek provides services for not only the residents of Carolina Bay but also the surrounding Wilmington area.

Assisted Living
Assisted Living offers residential accommodations with individualized assistance for things like administering medication, showering, preparing meals, and dressing. Residential choices include studios and one-bedroom apartments. The larger accommodations provide comfortable options for couples who wish to remain living together in one apartment. Residents enjoy a fulfilled life with daily opportunities for social and physical engagement.

Liberty Senior Living recognizes that each individual is unique, in both mind and body, and for that, we want to customize the experience of each individual that we serve.
Liberty Senior Living has taken steps to implement the teachings and training about dementia by Teepa Snow and the Positive Approach to Care methods. We now have access to a PAC Certified Independent Trainer Professional by the name of Holly Hines to guide learning and skills as supported by his or her certification types. While we are inspired by the content and methods included in the PAC Certification, our organization is solely responsible for the use and implementation of the approved content from PAC.
HOLLY HINES
Regional Dementia Care Coordinator and GEMMS Program Director
Holly Hines has been a part of the Liberty family for over 20 years, spending the majority of her time working directly with residents in our communities. Holly’s true passion in senior living was the engagement and care of our residents with cognitive diagnoses, such as Alzheimer’s disease or dementia. Her dream of exceptional programming for our memory support residents as well as immersive and comprehensive training for our caregivers, encouraged her to become a Certified Dementia Practitioner (CDP) through the National Council of Dementia Practitioners, as well as a Certified Trainer and Coach with Teepa Snow’s Positive Approach to Care.
As the program Director, Holly provides consistent training and education courses across all Liberty Communities, reinforcing Liberty’s mission of providing residents with the quality care they deserve from compassionate and competent caregivers.”
The GEMMS Stronger Together Support Group
Caregiving: One of life’s greatest gifts to give and receive! While never truer words have been spoken, we all know it can be both rewarding and challenging. As caregivers ourselves, we understand and want to offer a support group that will bring opportunities of education, inspiration and validation. We understand that knowing the resources available can assist in navigating those challenges.

Rehabilitation
When a patient requires short-term physical rehabilitation, the staff at Bradley Creek Health Center helps them find the strength to overcome setbacks following injury, illness, or accident. At admission, the clinical team starts with the mindset that all patients want to return to their prior living setting following rehab, and works with the patient, their physician and their families to build a foundation of skills that supports a safe discharge. This can include post-surgical rehabilitation, physical and occupational therapy, and wound care as needed.

- The Medicare Certified rehab gym hosts a therapy suite with a full kitchen, bathroom, and laundry room to allow therapists to assist patients in their return to managing every day needs once they return home.
- Modality programs such as diathermy, Tens units, ultrasound for pain management, strengthening, and increasing circulation are also available to aid in your recovery.
- Our car simulator assists therapists to work with patients for return of function to enjoy your return to community events, visiting friends, grocery shopping, etc.
Skilled Nursing
Offers around-the-clock nursing and rehabilitative services to address needs that arise from injury or illness. Our interdisciplinary team of skilled professionals make it their mission to restore residents to their highest potential.

Our rehabilitation and skilled nursing center has spacious private rooms that include television, wifi, refrigerator, and large private bathrooms with walk in showers. All of which assist in making your stay with us as comfortable as possible.

Carolina Bay residents will find peace of mind knowing they have seamless access to care knowing you can remain in a community where you already have an established support system. Our staff recognizes the individuality of each resident, and demonstrates complete discretion and respect at all times. Both short and long-term care are available at Bradley Creek Health Center we also offer palliative care services.
